Indication:
Used for the treatment of gastroesophageal reflux disease (GERD), peptic ulcers, Zollinger-Ellison syndrome, and other conditions involving excessive stomach acid production.
Pharmacology, Dosage & Administration:
Pantoprazole is a proton pump inhibitor (PPI) that works by irreversibly blocking the H+/K+ ATPase enzyme system of gastric parietal cells, reducing acid secretion.
Typical dose: 20 mg once daily before meals.
Duration of treatment may vary based on the condition, typically 4–8 weeks for GERD.
Interaction:
May reduce absorption of drugs requiring an acidic environment (e.g., ketoconazole, atazanavir).
Can increase blood levels of warfarin, phenytoin, and methotrexate.
Caution with clopidogrel as it may reduce its effectiveness.
Contraindications:
Hypersensitivity to pantoprazole or any proton pump inhibitors.
Caution in patients with severe hepatic impairment.
Side Effects:
Common: Headache, diarrhea, nausea, abdominal pain, dizziness
Rare: Hypomagnesemia, bone fractures (with long-term use), and allergic reactions
Pregnancy & Lactation:
Classified as Pregnancy Category B – No evidence of risk in humans, but should be used if clearly needed.
Caution is advised during lactation as it may pass into breast milk.
Precautions & Warnings:
Long-term use may lead to vitamin B12 deficiency, magnesium deficiency, or increased risk of osteoporosis-related fractures.
Monitor for signs of Clostridium difficile infection in patients with persistent diarrhea.
Should not be used for immediate relief of heartburn symptoms.
Overdose Effects:
Symptoms may include confusion, drowsiness, blurred vision, rapid heartbeat.
Treatment is supportive and symptomatic.
