Ingredients:
Telmisartan 40 mg
Direction of Use:
Take exactly as prescribed by your physician.
Swallow whole with water, preferably at the same time each day.
Can be taken with or without food.
Indication:
Treatment of essential hypertension (high blood pressure)
Cardiovascular risk reduction in patients at high risk for heart events
May be used in hypertensive patients with type 2 diabetes to reduce progression of kidney disease
Adjunct in heart failure management in select cases
Pharmacology:
Telmisartan is an angiotensin II receptor blocker (ARB). It selectively blocks the binding of angiotensin II to the AT1 receptor, thereby:
Preventing vasoconstriction
Reducing aldosterone secretion
Decreasing blood pressure and overall cardiovascular load
It does not inhibit ACE or affect bradykinin metabolism, which may result in fewer cough-related side effects.

Dosage & Administration:
Typical starting dose: 40 mg once daily
Maintenance dose: 20–80 mg once daily, based on response
Dose adjustments may be necessary in elderly, hepatic impairment, or volume-depleted patients
Do not discontinue abruptly
How to Use:
Take orally with a full glass of water.
Can be taken with or without meals.
Continue even if you feel well—hypertension often has no symptoms.
Do not skip or double doses.
Interaction:
Potassium-sparing diuretics, potassium supplements: May increase risk of hyperkalemia
NSAIDs: May reduce antihypertensive effect
Lithium: Risk of lithium toxicity may increase
Diuretics or other antihypertensives: May enhance hypotensive effect
Digoxin: Slight increase in plasma levels may occur
Contraindications:
Hypersensitivity to Telmisartan
Pregnancy (2nd and 3rd trimesters)
Severe hepatic impairment
Biliary obstructive disorders
Concomitant use with aliskiren in patients with diabetes

Side Effects:
Common: Dizziness, fatigue, back pain, sinusitis
Occasional: Hypotension, hyperkalemia, increased creatinine
Rare: Renal dysfunction, angioedema, liver enzyme elevation
Pregnancy & Lactation:
Pregnancy: Contraindicated, especially during the 2nd and 3rd trimesters (risk of fetal harm)
Lactation: Use with caution; not recommended unless clearly needed
Precautions & Warnings:
Monitor renal function and serum potassium, especially in elderly or those with kidney disease
Use cautiously in volume- or salt-depleted patients
Not recommended as first-line in patients with bilateral renal artery stenosis
Avoid alcohol as it may enhance hypotensive effect
Overdose Effects:
Symptoms: Marked hypotension, dizziness, bradycardia
Treatment: Supportive care, IV fluids; Telmisartan is not removed by dialysis
